What the smoke does
Combustion is where the majority of smoking’s acute harm originates. Burning tobacco produces over seven thousand chemical compounds, including carbon monoxide, tar, formaldehyde, benzene, and at least seventy confirmed carcinogens. These compounds cause lung cancer, COPD, cardiovascular disease, and the cellular DNA damage that drives most smoking-related mortality.
Remove combustion as vaping and heated tobacco products do, and the majority of this toxic load is removed. This is the factual basis of the harm reduction argument for non-combusted nicotine products.
What nicotine itself does
Nicotine is not the primary carcinogen in tobacco smoke. But it is not harmless.
Cardiovascular effects. Nicotine raises heart rate and blood pressure, constricts blood vessels, and increases clotting tendency. These are effects that occur with every nicotine exposure regardless of delivery mechanism. For people with pre-existing cardiovascular disease, these effects are clinically significant.
Addiction. Nicotine produces and maintains the dependence that makes cessation difficult by restructuring the brain’s dopamine system, upregulating receptors, and creating the withdrawal state that drives continued use. The addiction itself produces harm in continued exposure to tobacco products, in the psychological and social costs of dependence, and in the health consequences of the substances nicotine drives people to keep consuming.
Fetal development. Nicotine crosses the placenta and affects fetal brain development independently of combustion byproducts. This applies to NRT and vaping during pregnancy, not just cigarette smoking.
Potential cancer promotion. Emerging evidence suggests nicotine may promote tumor growth in existing cancers through receptor activation, though it is not itself a primary carcinogen in the way tobacco combustion products are.
The practical implication
For a current smoker choosing between continued smoking and NRT or other non-combusted nicotine delivery, the harm reduction benefit of removing combustion is real and substantial. NRT nicotine is significantly safer than smoked nicotine.
For someone who has successfully quit nicotine entirely, reintroducing nicotine through any mechanism, including “safer” delivery, reactivates the dependence circuit and restarts the health risks nicotine itself produces.
The goal is not safer nicotine delivery. The goal is no nicotine, which is when both the combustion harm and the nicotine harm end simultaneously.
