WHAT IS THE DIFFERENCE BETWEEN A LAPSE AND A RELAPSE?

The answer: A lapse is a single smoking event during a quit attempt. A relapse is the return to regular smoking. The difference between them is not the cigarette; it is what happens next.


The definitions

A lapse is one cigarette- bounded in time, after which the quit attempt continues.

A relapse is a trajectory- the lapse becoming the first cigarette of a resumed smoking pattern rather than an isolated slip within an ongoing cessation.

Both begin identically with the same cigarette, at the same moment. What makes one a lapse and the other a relapse is entirely what happens after that cigarette is smoked.


The abstinence violation effect – the mechanism that converts lapse to relapse

After a lapse, the person experiences guilt and then a specific cognitive interpretation of that guilt: “I have failed. I am a smoker. I have undone everything. I might as well smoke.”

This is the abstinence violation effect- the all-or-nothing framing that converts a single event into evidence of total failure. It is not accurate. But when it occurs unchallenged, it produces the emotional state in which continued smoking feels both justified and inevitable.

The cigarette did not cause the relapse. The uninterrupted abstinence violation effect did.


What a lapse actually means

Clinical information, not moral failure. It reveals precisely which circuit was not yet sufficiently extinguished to resist the specific cue that triggered it under the specific conditions present. That information, applied to the next attempt, makes the next attempt stronger at exactly the point where this one failed.


The response that keeps a lapse a lapse

Stop smoking immediately, and the lapse ends with the cigarette that was smoked. Name the abstinence violation effect out loud. Identify the specific circuit- what cue, what context, what conditions. Return to the protocol now, not tomorrow, not after one more cigarette. Tell one person.


The one thing to hold onto

A lapse is a cigarette. A relapse is a decision made in the moments after the cigarette, in the response to it.

The cigarette does not make the decision. The response does.

Name it. Return immediately. One cigarette is one cigarette, it becomes a relapse only if the response allows it to.
