HOW OLD DO MOST SMOKERS LIVE?

The answer: Smokers die on average ten years earlier than non-smokers; one of the most consistent findings in epidemiology, and cessation at any age recovers some or all of those years. Also, the quality of life is poor during the lived years for smokers.


The ten-year finding

Doll and Peto’s fifty-year prospective study of British doctors (the most comprehensive long-term smoking mortality data available) established the ten-year figure clearly:

Smokers who continued throughout their lives lost approximately ten years compared to lifelong non-smokers.

The recovery by cessation age:

  • Quit before 35- virtually complete recovery. Approaching non-smoker life expectancy.
  • Quit at 40- approximately nine years recovered.
  • Quit at 50- approximately six years recovered.
  • Quit at 60- approximately three years recovered.

What the average conceals

Heavy smokers lose more than ten years. Light smokers lose fewer, but not proportionally fewer, because cardiovascular risk does not scale linearly with cigarette count.

Smokeless tobacco users lose approximately five to seven years- less than combusted tobacco, but significant. Bidi smokers likely face outcomes at least equivalent to cigarette smokers given the higher carcinogen delivery per smoke.


How smoking takes the ten years

Cardiovascular disease- approximately forty percent of smoking-attributable mortality. Heart attack and stroke occur earlier and more suddenly than cancer.

Cancer- approximately thirty-five percent. Lung cancer is the largest contributor, but smoking causes cancer in at least fourteen organ systems.

Respiratory disease- approximately twenty percent. COPD’s progressive disability reduces quality of life in the years before death.


The quality of life dimension

Life expectancy figures measure years, not how those years are lived. The decade smoking removes is not only at the end of life. It is distributed throughout middle and older age as breathlessness, reduced mobility, cognitive decline, and medical dependence that statistics do not capture.


The one thing to hold onto

Ten years is not an abstraction. It is a decade of grandchildren, of retirement, of the life the final decade of a non-smoker’s life contains.

Cessation before forty recovers virtually all of it. Cessation at any age recovers some of it.

The decade is not gone until smoking continues until it is.
